The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is reported to be extremely contagious. The virus spreads through person-to-person contact and/or by contact with contaminated surfaces and objects, and even possibly in the air in the form of droplets. People can be infected and show no symptoms and therefore spread the disease. There are known vaccines that many of the U.S. population have received to prevent spreading and contracting of the disease. In addition, mask usage and washing of hands are highly encouraged to prevent spread and contracting. Evidence has shown that COVID-19 can cause serious and potentially life-threatening illness and even death. The Dominique Broadway Brand, LLC (“Company”), its respective representatives, affiliates, licensees, successors and assigns (collectively and individually a “Released Party”) cannot prevent you or members of your household from becoming exposed to, contracting, or spreading COVID-19 while utilizing the Company’s services, premises, or by attending Company sponsored events. It is not possible to prevent against the presence of the disease. Therefore, if you choose to utilize Company’s services and/or enter onto Company’s premises and/or attend Company sponsored events, you may be exposing yourself to and/or increasing your risk of contracting or spreading COVID-19.
ASSUMPTION OF RISK: I have read and understood the above warning concerning COVID-19. I hereby choose to accept the risk of contracting COVID-19 for myself and/or members of my household in order to utilize Company’s services, enter Company’s premises, or attend Company sponsored events. These services are of such value to me that I accept the risk of being exposed to, contracting, and/or spreading COVID-19 in order to utilize Company’s services, premises, and events in person.
WAIVER OF LAWSUIT/LIABILITY: I hereby forever release and waive my right to bring suit against Finances Demystified LLC and its owners, officers, directors, managers, officials, trustees, agents, employees, or other representatives in connection with exposure, infection, and/or spread of COVID-19 related to utilizing Company’s services, premises, and events. I understand that this waiver means I give up my right to bring any claims including for personal injuries, death, disease or property losses, or any other loss, including but not limited to claims of negligence, and give up any claim I may have to seek damages, whether known or unknown, foreseen or unforeseen.
CHOICE OF LAW: I understand and agree that the law of the State of Delaware, United States of America, will apply to this contract.

- I hereby give and grant in perpetuity to The Dominique Broadway Brand, LLC (“Company”), its respective representatives, affiliates, licensees, successors and assigns (collectively and individually a “Released Party”), the right to use, publish and copyright my participation, likeness, photographs, video, name, voice, and any testimonial statements (collectively the “Released Information”) in all media now known or later discovered, for the purpose of advertising and promotion of Company, and event (“Event”) taking place on May 14, 2022. 2. I agree that all Released Information provided by me to the Company are owned by them and that they may use or not use, nor be obligated to use, the Released Information at their discretion. 3. I agree that Company has the unfettered right, among other things, to use, exploit and distribute the Released Information (including, without limitation, any photographs, videos, or other material provided to Company by me), in any and all media or formats, throughout the world. 4. I agree that no advertisement or other material need be submitted to me for any further approval and the Released Parties shall be without liability to me for any distortion or illusionary effect resulting from the publication of my Released Information. 5. I understand and agree that I shall receive no compensation for the Released Information or its use. 6. I further agree that the Released Parties will have the right to attribute the Released Information (or derivative versions of the Released information which have substantially the same meaning) to me, which is an expression of my personal experience and belief. 7. I understand and hereby agree that I have no right to revoke the use of the Released Information; therefore, releasing all personal and legal privileges and rights in the Released Information to Company. 8. I hereby agree to not resale my ticket purchased for the Event. I also understand that I am not due a refund if I cannot attend the Event. 9. I further agree to indemnify and hold Released Parties harmless from all liabilities, claims, demands, costs, expenses and attorneys’ fees incurred as a result of any such suits, causes of action, claims, demands and entitlements filed by me or my assignees. 10. I hereby release and discharge the Released Parties from any and all liability arising out of or in connection with the distributing, publishing, transmitting by any means or otherwise using the above-mentioned Released Information.
